          When you work with personal data and a healthcare startup in 2022, you need highly secure infrastructure so the data will not be compromised by anyone, including your team or any third parties. To make sure it is safe and sound, the project infrastructure usually needs to be rebuilt using up-to-date technologies and approaches towards data protection.
          The project being discussed in this case study is a Healthcare App*, a healthcare IT startup, that allows users to track their well-being and generate reports so their physicians can see their symptoms over time. Customers for this Healthcare App include insurance companies that are interested in helping their members at early stages of medical issues to reduce costs which makes it also a B2B healthcare startup. The most important thing for these companies is the protection of their members’ data. The developer of the Healthcare App must complete a detailed IT Risk Assessment and ensure it is SOC2 Type 2 certified before an agreement is completed. Therefore, our task is to rebuild the project infrastructure so it could meet their requirements.

            The project was initially developed with a custom code written in Drupal 6. However, this solution is not that good or efficient in terms of data protection. The custom code had a lot of weak spots and could hardly incorporate any new functionality, meaning it would be difficult to meet the insurance companies’ requirements mentioned above. Our first step to make it secure was to redesign the infrastructure and make it based on micro services written in Symfony which is a PHP framework known as one of the most secure solutions around PHP out there. Micro services architecture itself brings an opportunity for developers to separate services, work on those pieces independently and keep them up-to-date which increases entire application availability.
The second step was to arrange a compliance structure to make sure that all the processes and general workflow in the team matched the requirements. The structure was based on HIPAA (US) and PIPEDA (Canada) laws for private-sector companies working with personal data. An updated workflow included deployment process involving mutual code-reviews and several roles to accept recent changes before they went live which is important both for already operating or new healthcare startups.
The last step of the action was to get certified in terms of security by a specialized third-party company in accordance with SOC2 after integrating all the changes listed above. For this, the third-party company hired an external consultant to perform an application penetration test - this involved a set of attempts to hack the application and a report of all weak spots that were identified in the application. The majority of them were discovered due to using elements of Drupal 6 in the project, however, it was not possible to get rid of it completely as some of the core elements required it. To undergo further certifications, we rewrote the majority of the project with Symfony as planned and left Drupal 6 only for our client’s purposes such as admin panel and a couple of other important features. Other parties did not have access to Drupal 6 features anymore.

            After integrating all of the changes and marketing campaigns, the project came up with a range of new metrics listed below:
          * The application used to have unplanned downtime at least once a month before integrating new infrastructure, but it has not been down since the change.
          * API-calls are now processed 5 times faster than before.
          * Code quality got optimized after bringing a new workflow based on Agile and including development, QA and review stages with different roles. Releases used to take several months to commit, now they take only 2-3 weeks.
          * During the past year, the application got 15,000 more users.

          The application*, described in the case study and known to be a sport startup, is targeted on the fitness market in Thailand and Australia with a goal to go global later once the MVP proves its business idea and economic efficiency. Its main functions are to provide users with an opportunity to do sports and yoga exercises at home and to allow them joining challenges to share their achievements with each other. The main revenue channel is based on the subscription system to get access to the content of the application which can be edited by admin and editors. Our task was to build the MVP of the application with all the functions listed above and release it in both AppStore and Play Market so our client could test it in both countries to see if there was a demand meeting their goals.

            To make a sports tech startup from scratch, you need at least 2 phases — planning and development itself. As for the planning stage, our sales manager and system analyst gathered all the necessary requirements and provided the client with a set of user-story to estimate with a developer. We stick to the user-stories approach as it can represent all the necessary parts of the application for all the roles on the project and can be used either by a client to see if everything that they want is being considered or a developer to estimate the project and split it into milestones or iterations. We also discussed channels of communication, forms of reports and meetings so it all gets clear before getting further. Once it was all sorted out, we got ready to get to the development stage itself.
During the development stage, we assigned an account manager to the project so our client could get regular updates regarding the progress and a person to communicate with if there were any questions. We use a range of tools for project management: for example, Redmine and Coda are our go-to choices for planning and task management, Slack, Skype and Zoom are great for regular calls for updates. For this project we used a custom template created on Coda and Zoom to have a call once in two weeks regarding the progress for each milestone.
As for the technical part, the sport startup consisted of a mobile application for both iOS and Android users, written in React Native, and a Laravel back-end providing owners with all the necessary API endpoints for the application. We decided to go with this stack due to the product: those technologies allowed our client to build the application quite fast and were  flexible in terms of changes in MVP if necessary, which is quite important when it comes to testing your business idea. We used Vimeo to store the video and the service interacted with the back-end via API. Those solutions allowed us to build an application that would both be efficient in terms of loading and working with a high number of users on the application. Back-end also allowed admin and editors to add and edit the content, manage users and their subscription status, set requirements for achievements in the challenges system and split the workout videos into routines so the user could easily navigate them. The application supported a custom player developed by us which allowed users to use routines for a better navigation mentioned above which could be managed by admin and editors either in admin panel or in Vimeo. We also integrated a language system to the back-end as the application supports two languages even in MVP.

            As a result, we published the application on AppStore and Google Play so it could serve as a beta version and be tested by several thousands of users at the moment to make the sport startup go live. The case study is to be filled up once the application is over the beta testing phase and out on the market to add all the necessary metrics.

            The typical workflow in the system looks as follows: Manager logins into the system using web or mobile application, creates a task for some employee or group of employees. Employees get notification to their mobile devices about tasks they were assigned to. These notifications are implemented using web socket server. Tasks can be To-Do lists with expiration time or periodicity, and such lists can be hierarchical.

            During our work, we designed and implemented our own database and ORM for every necessary entity using CodeIgniter.
Accordingly to these entities we implemented all web application menus and interface elements, using JS, jQuery and CSS. All these stuff allows to perform necessary actions for all user types.
Another big part of work - is rewriting PHP web socket server on Node.js. We designed it like some kind of 'proxy' server, which calls different web app model's methods depends on mobile requests. It makes possible to avoid duplicating calls to database on web socket and typical web server parts. Also, it allows to improve common server performance and increase connections number. All interaction between mobile and server side executes using special API.

            The believed that an efficient solution that would save the company their budget and allow to generate more revenue by simply providing teachers with an opportunity to use their supplements online was Drupal. This CMS takes into consideration a lot of technical details that need to be resolved before starting developing the product such as different roles or users and their permissions, content creation, management, distributing and integration, advanced visualisation, day-to-day services integrations (for example, email or payment ones). The elements listed above are usually ready to use in Drupal — some of them do need some initial set-up, but you do not have to develop anything from scratch. Only the smallest number of tasks needs to be done in a custom way and is totally up to the level of customisation you would like to have.

After developing the platform itself with all the elements listed above, we also decided to introduce a new content management tool as the business model of the company was mostly based on providing teachers with content online. Our team introduced new advanced content creation forms that had a lot on common with WordPress and AEM solutions as the company used to work with these CMS. On the other hand, they were much simpler to use for editors and provided them with some hints regarding creating and distributing their content. We also automated and simplified working with images and videos elements by using this tool so editors could post and edit these elements in the easiest way. This solution was brought both by already made solution presented in Drupal and the level of customisation that we managed to bring by combining them together.

            Even though Drupal is considered to be quite heavy and slow in terms of performance, it all comes down to the optimisation. As a result, we managed to achieve the following results:

          * We optimised Drupal so it would not load up the website, but would be able to undergo a lot of users and work without being down.

          * The team intended to work and support on the website decreased by 20% as the solution did not need a lot of developers to maintain it in the future. 

          * The company was able to attract more clients and go global, especially during and after pandemic when online education became a trend.

          * The net profit of the company during the first year after rebuilding the platform came out as a 6 figure number in US dollar.

            Connell Guides creates accessible guide books that help students of different ages better understand great novels, poems and plays on the main English syllabuses better. The CG (Connell Guides) subscription service provides access to e-book guides, literary briefings and videos of text discussions, produced by CG itself. Over time Connell Guides asked us to help improve the website several times. CG website was initially built by another team using Drupal 7. Security and UI issues were left without attention, also with time business development purposes requested changes on the website.

The original website was built using Drupal Commerce and the main purpose was to sell guide books created by CG. It was privately funded and built badly: the main issue was underperforming functionality which was the first task. It was a huge blocker for business development.

After a while CG added a subscription service (image below) for students so that they would not need to order printed guide books and could access other helpful content created by CG. However, they were experiencing security issues, and faced a huge amount of spam registration accounts and XSS attacks that were badly affecting the business that occurred to be the second task.

            Within a short period of time we carried out a successful refactoring that allowed CG to
develop their business. We started by updating the backend of the website to the latest Drupal version available to solve security issues. To eliminate spam we have simply added Captcha, and detected and
corrected an error related to XSS attacks. As a result of these changes, a smooth subscription management process was established which helped CG to continue to expand their business.

To make the website supportable, secure and good-looking we implemented a number of
changes:

* Migrated the website to a new domain

* Configured Google Analytics

* Updated Drupal core and modules

* Configured roles and permissions for the subscription service

* Updated website’s design

We used the following technologies to bring the changes: Drupal 7, HTML, Javascript, CSS, GIT(Bitbucket) and AWS.

            Prepclass is a Nigerian EdTech (educational technology) company that focuses on connecting potential learners with skilled tutors in a variety of areas, including nursery, formal and informal education, etc. 

Although Prepclass tutors specialise in different areas, most of these areas have standardised tests as the final exam. Because of that a significant part of the educational process is based on practicing the previous years’ tests. The problem is that they usually have different formats, leading to a lot of paperwork and a custom organisational process when tutors need to verify results.

The task was to create an application that allows Prepclass to automate and simplify such communication between tutors and learners when practising standardised tests. Also the application should provide easy access to educational courses, as well as a convenient way for learners to ask questions and for tutor to answer them. The plan was to deliver the product in two phases - the initial MVP and the main part of the project.
        </div>
        <div title="Action">
            When we started working on phase one, the main issues were MVP requirements, a limited budget and deadline constraints. After several rounds of requirements investigations and discussion, we proposed an actual MVP list of features and an implementation plan. After receiving the client’s approval, we delivered a fully working application with an administrative site in 4 weeks.

Prepclass needed the ability to use dynamic formulas (similar to LaTeX format) in courses and
exam questions, but there was no existing solution for React Native, only limited libraries for
browser use. We made a custom module which renders such formulas in React Native and implemented a
special plugin for WYSIWYG editor on the administration site. As a result, tutors were able to type any formula in a suitable format, thus the amount of time required to add the course into administrative site or edit it decreased to simple formula text editing.

Our client also needed to integrate a local payment system based on cellular network MTN
Nigeria to the application, but the cellular operator had neither SDK, nor comprehensive
documentation for their payment API. We spent two weeks communicating with cellular operator technical support, investigating the existing version of their API description to get our own full and valid API documentation. Then we built a client SDK for that API both for the React Native app and the administrative site. Eventually the payment system was integrated to the application, and users now have the option to pay through the familiar service. The process from the start of investigation to the release of the final version of the SDK together with the application and the admin site took 4 weeks.

In phase two we added more features and improved the application:

* simplified the process of adding courses and made it faster by implementing our own
custom module

* added analytics for both - users and tutors - to track progress and identify users’ weak
spots

* added a Q&A section where users and tutors can communicate

* added an integration with the local payment system

To bring our solution, we used the following technologies: React Native, React, Redux, Flow, Laravel PHP 7, CSS, SASS, HTML, JS, Jquery, Bootstrap

            The MVP was delivered within the set time and budget and in general was very successful by presenting it our client was able to get funding for further phases.             It is a well-known fact that some businesses out there today have more than one website to serve their goals. HIMSS used to be one of these companies — they had more than 20 diverse websites and they wanted to achieve the point at which the design of each website would inspire confidence and be associated with the main website. To make this, the company presented the design guideline that contained all the visual components of the website that needed to be developed by us into a universal Drupal theme so all the parties could have access to it and could apply to any new website they had coming with minor updates from their side.

            We decided to come out with a unique but universal approach at the same time — our team decided to develop just the only one theme with basic interactive widgets which would have been easily edited and applied to a new website. This theme was intended to be located in a separate repository so all the developers from HIMSS could have access to it to use whenever they needed it. As a result, all the website had the same structure and could be edited at any time in a simpler way. To make any adjustments, they simply had to create a new CSS file, using the special naming proposed by us, but theme components remained the same which eventually helped the company to reduce hours for websites development. 

It is important to highlight the fact that it is quite impossible to create a universal theme from scratch at once — it takes time and user experience to identify all the components that has to be integrated in the solution. That is why we assigned a developer to this project — once they identified any new elements, we updated both theme and guidelines on Confluence right away so HIMMS had any new information and opportunities for their products development. The developer also made sure that all the new elements were compatible with the rest of them by conducting a range of tests.

            As a result of developing a universal theme, we managed to help HIMSS to achieve the following: 

1. The company now was able to develop a range of new websites with less hours. Basically, it all came down to the designer at that point — they just had to set up the color scheme and the new website was ready to go. 

2. Annual redesign of all current website used to take several months of team work. With our solution they just needed to have one developer and one week of their time to do the same task which reduced the costs up to 10-12 times. 

3. Sometimes the task was so easy so it was not even necessary to assign a developer. This allowed HIMSS to reassign their resources which eventually led the team to develop a multi-site instead of a range of separate ones. This made the development process even easier as the website now had the same code base and didn’t need a range of updates on a separate solutions.

            One of the most frequent practice today for companies that have more than one website is to gather data from all the websites to collect and analyse just by using the only one source. The same approach was used by HIMSS — the company had more than 20 conference websites that had different data about users’ behaviour and needed to store the data in one space to transfer it next to their analytics service. The task was to develop the unified storage so the company could store data from all their websites just in one place.

            We came up with a solution to develop the unified storage as a separate website based on Drupal which would use its API to collect data from other websites. In other words, we developed a micro service to collect and store data from all HIMSS websites.

            The issue occurred later not with our solution but with the service the company used to analyse the data. Our task was basically to develop a unified data storage that would eventually transfer the data to that analytics service, however, the issue happened with the service itself. In 2018 the conference was attended by a record number of users which was about 45 000 just in 7 days. Once we started to transfer their data to the analytics service, it simply broke down as it could not keep up with that amount of load from the storage side. The service was used by the company as it had an advisory feature based on users’ behaviour which was a crucial point for HIMSS. Realising the fact that the server would not be able to work as expected, we managed to update the unified store so it would: 

            * store the data for much longer than before as it might have been needed in the future after the conference. 

            * serve as the previous analytics service so the company could continue operate at a conference as planned.

            As a result, not only did we manage to develop a unified data storage so the company could keep their data from different sources just in one place, but also replace analytics services that did not work as expected. Moreover, it took us just one night to develop the latter feature and users did not even experience any issues with the main conference website while working on it that eventually allowed to sustain behaviour and revenue metrics on the website as planned.

            The Daily Meal website has around 30-50M pageviews, 2.5M unique users on average per month, more than 250GB of image assets and 1.7M+ recipes. It is very high-loaded. To provide good speed, maximum uptime and security, the hosting conditions were improved constantly. Over time this resulted in an overpriced hosting.

            Most of the content on the site are static articles with a lot of assets and ads.

Thus in terms of basic site performance we concentrated our focus on the proper use of caching, starting from web framework caches and leading up to CDNs. Having a proper caching, expensive and high-performance server is not a necessity, so we moved the site from customised AWS based platform to Pantheon, which is specialised for Drupal and WordPress. 

To increase the robustness of the website we have built a disaster recovery platform on AWS that replicates the site infrastructure. It is disabled most of the time and the client is not charged. When the main hosting fails the client can turn it on and swap the hosting temporarily.

Assets we have moved to a separate specified server that only works for providing assets. Unfortunately, all out-of-the-box solutions were expensive, so we have provided our own based on the open-source application Thumbor.

In terms of ads, we have very precisely tuned the way ads load and appear on the site, investigated existing ad platforms to choose the fastest and the most revenue efficient options and are constantly monitoring and supporting them.

            As a result of the work we achieved the following outcomes:

            * Monthly hosting charge has been decreased by 4 times (400%). 

            * Moreover, the development process has become cheaper and more robust because specialised hosting is now partially responsible for maintaining deployment tools. It helps to provide ongoing support of the website with only 2-3 developers.

            * The overall website performance has also increased from 50% to 80% of page load (only HTML) finished in 0.5 seconds or less, and from 20% to 60% of full page loads (including scripts, ads, assets, and so on) finished in 7 seconds or less.

            There are cases when a release can cause minor issues, such as JavaScript errors, wrong ads display and more. These issues can quickly affect the Google algorithm and number of users. Moreover, they can also affect user behaviour and conversion. We needed a tool to help us find and solve these issues as quickly as possible.

            We came up with a dashboard that would show the most important SEO metrics. The idea is to be able to know whether the site performs as planned. A dashboard with graphical and tableau representation is built to monitor the metrics.

Given predefined / planned metrics for the site, such as pageviews, sessions, conversions (of a various kind: video plays, ad views, scrolls to the site bottom, form submissions) and the set of site sections (e.g. Home Page, Video Network, Content Archive landing pages) managers are able to see if overall site or just a specific section satisfies, exceeds or does not match the planned values within the selected period of time (e.g. last day, week, month, month to date).

The data is primarily GA but it is also possible to use internal event tracking to see things fast and in one place.

Another case is to send this data daily and weekly in form of an email newsletter to authorised staff members. It should include a comparison of the data between the current and previous weeks (for weekly reports) and between the current and previous days and daily average (for daily reports).
